2012 Registration

Registration Dates

Registration will be held on the following dates and times in the Altamont Board Room:

21 January 2012, from 9am to 3pm
28 January 2012, from 9am to 3pm
31 January 2012, from 5:30pm to 7:30pm
02 February 2012, from 5:30pm to 7:30pm
09 February 2012, from 5:30pm to 7:30pm

Cost of Registration

There are 3 fees for registration:

Registration is: $95 for the first child, $60 for second, and $45 for each additional child.
Refundable field workday fee: $40 per family.
Fundraiser, pick one of these three options:
1. Take a box of candy, sell it and bring the money back to Altamont.
2. Pay $60 for a box of candy, sell it and keep the money to replace the $60 you paid.
3. Pay $40 and you don’t have to deal with the candy.

How do I get my $40 workday fee back?

Come out on our field workday; sign in; work for 2 hours; sign out and get your refund. See the Calendar of events for dates and times

What if I cannot afford the registration fee?

Altamont has a scholarship program whereby you take 3 boxes of candy for the first child being registered, and 2 boxes for each child after that, sell them, and bring the money back to Altamont. Under this program, all fees are waived but you must come and work for 2 hours on field workday. Field workday is from 9am to 4pm, so you will need to arrive by 2pm to be able to put in 2 hours of work.

Paperword Needed

On the day of Registration you must bring the following with you:

Players Birth Certificate
3 Proofs of Residency

Please look at our boundry map to see if you are in the Altamont League Boundries. If you are not in our boundries, this map will show what league boundries you are within.

2012 Registration Forms

All players must register before playing, if your child played last year with Altamont we have a registration form pre-filled out for you, so you will not need to download one. If your child did not play with Altamont last year please download and fill out this form, and bring it with you to registration.

All players will need a Medical Release form filled out, signed and given to the coach prior to the first practice.

Waiver Forms

The three following forms are Waivers for players:

Wants to play for Altamont but does not live within the boundries.
- Residency Waiver Request Form
Lived in Altamont boundries but has moved.
- Report of Players Claimed under Regulation II(d)
Lived within Altamont and parent served Altamont for two years on the board of directors, as a manager, or coach.
- Regulation IV (h) Waiver Claim Form

Birth Certificate Form

If you do not have the childs birth certificate this form must be filled out.

Statement in Lieu of Acceptable Proof of Birth
